OVERVIEW

Section 204 of the Tax Relief and Health Care Act of 2006 (TRHCA) mandates a demonstration in up to 8 states to provide targeted, accessible, continuous and coordinated family-centered care to Medicare beneficiaries who are deemed to be high need (that is, with multiple chronic or prolonged illnesses that require regular medical monitoring, advising or treatment.) On September 25, 2007, CMS selected Mathematica Policy Research to provide assistance in the design of the Medical Home Demonstration. Implementation is expected in January 2010. The demonstration will take place in eight states, involve urban and rural practices, and target chronically ill Medicare patients. CMS will start recruiting physician practices for the project in January 2009.

What Is an Audioconference?

It is a live event that includes session handouts, an interactive question and answer period, and access for an unlimited number of participants at each call-in site. However, only one site can be offered per registration. It is impermissible for an organization to purchase one registration and to operate multiple sites through an internal telephone system. During the live Audioconference participants will be able to ask questions and make comments. The Audioconference faculty has agreed to take follow-up questions via email. Audioconferences are simple, accessible, cost effective and reach a broad audience locally.
