FACULTY BIOGRAPHIES

Melinda Abrams

Melinda Abrams is senior program officer at The Commonwealth Fund and directs the Patient-Centered Primary Care program. Since joining the Fund in 1997, she has worked on the Fund's Child Development and Preventive Care program, the Task Force on Academic Health Centers, and the Commission on Women's Health.

She helped conceptualize and launch the Fund's Assuring Better Child Health and Development (ABCD) initiative in 1998-99, which awards grants to state Medicaid programs for innovation approaches to services for low-income, young children. Ms. Abrams has sat on advisory committees of various national organizations, including the National Initiative for Children's Healthcare Quality, the Child and Adolescent Health Measurement Initiative and AcademyHealth. Prior to coming to the Fund, Ms. Abrams directed community outreach and public education activities for the Reproductive Rights Project of the New York Civil Liberties Union.

She holds a B.A. from Cornell University and an M.S. in health policy and management from the Harvard School of Public Health.

Jeffrey Brenner, MD

Jeffrey Brenner, MD is a family physician who has worked in Camden, NJ, one of the poorest cities in the country, for the past 10 years. Dr. Brenner attended Robert Wood Johnson Medical School where he helped start HIPHOP, the school's community outreach program. He completed a Family Medicine residency at the Swedish Health Center in Seattle, Washington. Dr. Brenner is faculty in the Departments of Emergency Medicine and Family Medicine at the Cooper Health System where he teaches medical students, is the faculty advisor for student community service projects, and conducts research on urban health conditions in Camden, NJ. His primary professional interest is ensuring that all families receive high quality, culturally competent, personalized family health care in urban, underserved communities. Dr. Brenner started and is the Medical Director for the Camden Coalition of Healthcare Providers, a non-profit organization committed to improving the quality, capacity, and accessibility of the healthcare delivery system in Camden. The organization runs a Citywide Care Management Project targeting "super utilizers" of the local emergency rooms and hospitals and a Chronic Disease Collaborative to help local primary care offices adopt the Chronic Care Model and become Patient Centered Medical Homes.

Paul Kaye, MD

Dr. Paul Kaye has been a pediatrician for 30 years and, since 1991, Chief Medical Officer of Hudson River HealthCare, a Joint Commission accredited Community Health Center network serving over 45,000 patients at 14 sites in New York State. Since 1998, Hudson River HealthCare has worked with the Institute for Healthcare Improvement and the Bureau of Primary Health Care to implement new models of primary care delivery. In 2007, its physicians received recognition from NCQA's Physician Practice Connections program.

Dr. Kaye is Co-Chair of the Pay for Performance and Medical Homes Workgroup of the National Association of Community Health Centers and represents NACHC on the AQA Alliance. He currently serves on the Technical Advisory Panel for the Commonwealth Fund's Transforming Safety Net Clinics into Medical Homes project, and is Medical Director of the Taconic IPA's Community Transformation Program.

He graduated from SUNY Upstate Medical Center in 1971 and did his pediatric residency at University of Wisconsin Hospital in Madison.
