OVERVIEW

One of the cornerstones of the medical home model is the notion of a care team that works closely together to ensure that patients have a continuous, healing relationship with all of the health care providers who might be responsible for their care. This is often described in terms of a primary care physician working with a Nurse Practitioner and/or nurse case managers to coordinate care with and for the patient, both inside the medical home and outside with any specialist referrals that are needed.

But there are very important and growing roles for a variety of non-clinical members of the medical home team, especially for patients with chronic conditions. These are the team members who may manage and track the patient registry, provide regular contact and encouragement in patient self-management, and help ensure access to community resources. This audioconference will highlight these new roles in a variety of different settings - a health care system, a citywide coalition effort, and a community health center or small practice.

What Is an Audioconference?

It is a live event that includes session handouts, an interactive question and answer period, and access for an unlimited number of participants at each call-in site. However, only one site can be offered per registration. It is impermissible for an organization to purchase one registration and to operate multiple sites through an internal telephone system. During the live Audioconference participants will be able to ask questions and make comments. The Audioconference faculty has agreed to take follow-up questions via email. Audioconferences are simple, accessible, cost effective and reach a broad audience locally.
