OVERVIEW
The process of transforming a physician practice into a recognized Patient-Centered Medical Home is neither linear nor standard. As previous reports have documented, in particular the so-called "Nutting Report" in the May/June Annals of Family Medicine, there are many paths to a medical home. The process is also difficult, involving acquisition or modification of IT systems, changes in office workflow and roles, changes in patient scheduling and management, and even cultural shifts. It is also a "journey" -- which connotes the need for continuous self-reflection and improvement -- rather than a "destination" that is reached when NCQA recognition is granted. This audioconference will highlight the challenges, successes, and current activities of two practices that are traveling on this medical home journey.
What Is an Audioconference?
It is a live event that includes session handouts, an interactive question and answer period, and access for an unlimited number of participants at each call-in site. However, only one site can be offered per registration. It is impermissible for an organization to purchase one registration and to operate multiple sites through an internal telephone system. During the live Audioconference participants will be able to ask questions and make comments. The Audioconference faculty has agreed to take follow-up questions via email. Audioconferences are simple, accessible, cost effective and reach a broad audience locally.
